When the form is sent back to the requester, please make a copy and send to the Parish Office. You keep the original.

You need to send in a copy of the processed Request for Criminal Record Check with this form.

When the form is sent back to the requester, please make a copy and send to the Parish Office. You keep the original.

You can go on the Archdiocesan website – http://www.archphila.org and check on the blue box on the right hand side of the first page – Victim Assistance and Safe Environment – click on the Safe Environment green box for forms.

There should be four forms on file in the Parish Office:

PA State Police Request for Criminal Record Check – good for five years

PA Child Abuse History Clearance – good for five years

Proof of attendance at Safe Environment Training

Standards of Ministerial Behavior and Boundaries signed acknowledgement
